Tokkuri, and how a machine pours warm sake
In Japanese the topper is a kae-tokkuri (替徳利), a spare for the tokkuri, the flask sake is traditionally poured from. In a machine like the Taiji warmer the topper plays the same role the tokkuri's neck does by hand: it directs the sake as the bottle is inverted and warmed, so each pour comes out controlled rather than glugging. It is a consumable part, which is why Taiji sells it on its own.
